Transfer paper of pension or certificate of pension payment;

3)身份證原件及復(fù)印件四張

A card of Identification Card;

4)學(xué)歷證明、職稱證書原件及復(fù)印件兩張

Certificate of diploma or title;

5)4張身份證照片(1寸藍(lán)底)

Photo(one inch)

6)近期體檢報(bào)告
